About the Job
As a Home Health Physical Therapist Assistant with All Medical Personnel, you will have the opportunity to make a meaningful impact on patients' lives in their own homes. About the Role
This is a full-time position that involves working independently in patients' homes, using evidence-based practice for assessment, clinical decision making, and coordination of care across the continuum. You will work collaboratively with an interdisciplinary team, including physicians, nurses, and social workers, to develop patient-centered plans of care that promote optimal health, well-being, and quality of life.

Duties and Responsibilities
* Assess patients and collaborate with the interdisciplinary team to develop patient-centered plans of care
* Provide skilled, intermittent physical therapy services to patients in their homes
* Instruct, coach, guide, and intervene to support patients and families in achieving the best possible outcomes
* Promote optimal health, well-being, and quality of life of home health patients and their families
* Practice independently in patients' homes, using excellent, evidence-based practice for assessment, clinical decision making, and coordination of care across the continuum
* Act as case manager and team lead for patients requiring home health therapy services when nursing is not involved or physical therapy is the primary patient need

Requirements
* Current license in Physical Therapy (PT) or ability to obtain prior to hire
* 1-2 years of experience in an outpatient rehabilitation setting
* At least two (2) years of physical therapy experience highly preferred
* One (1) year experience in home health or other health setting with geriatric emphasis
* Excellent customer service skills and ability to work without direct supervision
* Ability to maintain effective relationships with other healthcare professionals and develop skills in use of technology used in home care settings
* Strong knowledge of physical therapy assessment and treatment methods, physical therapy modalities, and equipment
* Must be able to provide care appropriate to the age of patients served on the job

Compensation
The estimated salary for this position is $63,000 - $85,000 per year, based on industry standards and location.
