Licensed Vocational Nurse

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team at Pine Creek Care Center. As an LVN, you will play a vital role in providing high-quality patient care to our residents.
Responsibilities:- Implement the established plan of care for each assigned group of residents, ensuring their physical, psycho-social, and spiritual needs are met.
- Administer medications, perform treatments, and provide resident/family education as directed.
- Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
- Monitor and report resident condition changes to the Registered Nurse, attending physician, and Director of Nursing.
- Provide treatment administration, including wound care, indwelling catheter care, and enteral feeding tube management.
- Participate in the identification of resident/family educational needs and provide individualized teaching.
- LVN graduate from an accredited School of Nursing.
- Current, valid LVN licensure to practice in the state.
- Minimum 6 months prior LVN experience in a general acute care hospital or acquired equivalent competency.
- Perform assigned resident care duties in a manner that provides for the physical, psycho-social, and spiritual needs of the chronically ill and technologically dependent resident.
- Complete initial and ongoing assessments by gathering data in a timely manner.
- Correctly differentiate between normal and abnormal clinical findings and intervene accordingly.
- Work collaboratively with the resident/family and interdisciplinary team members to develop an individualized plan of care.
- Demonstrate sound clinical judgment in the implementation and evaluation of the nursing aspects of interdisciplinary resident care plan of care.
