About the Role

We are seeking a highly skilled and compassionate Health Coach to join our team at One Medical. As a Health Coach, you will play a vital role in supporting our patients with meeting their preventive, chronic, and acute care needs.

Key Responsibilities
  • Establish Trusting Relationships: Develop and maintain strong, supportive relationships with patients and their families, advocating for their needs and preferences.
  • Coordinate Care: Collaborate with healthcare providers to ensure seamless care coordination, including scheduling appointments, coordinating referrals, and sharing information with the patient's care team.
  • Monitor Health Outcomes: Work with providers to monitor the health of a panel of patients, ensuring they are up-to-date on preventive measures and addressing any health concerns.
  • Provide Education and Support: Offer patient education on chronic disease management and coach patients using an action-planning model based on motivational interviewing techniques.
  • Participate in Daily Operations: Contribute to the daily operations of our primary care practice, including answering phone calls, responding to emails, and ensuring the general upkeep of the clinical space.
Requirements
  • Experience: At least 1 year of experience in a high-touch customer service or patient-facing role in a healthcare setting.
  • Communication Skills: Strong written and verbal communication skills, with the ability to effectively communicate with patients, providers, and other healthcare team members.
  • Teamwork: Experience working on collaborative, diverse, and feedback-driven multi-disciplinary teams.
  • Adaptability: A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges.
  • Technical Skills: Proficiency in computer technology, including typing, navigating the internet, and using multiple software systems simultaneously.
