Job Summary:

VCU Health is seeking a Registered Nurse to join our team in the Continuum Integration Center. As a Nurse Clinician, you will be responsible for providing high-quality patient care in a Hospital at Home setting. This is an exciting opportunity to be part of a new program that offers innovation and professional growth.

Responsibilities:

• Provide routine and urgent in-home visits to patients in the Hospital at Home program
• Assist with setting up patients in the home after transfer from the brick-and-mortar hospital
• Perform telephone triage and coordinate care with the Hospital at Home team and community partners

Requirements:

• Current RN licensure in Virginia or eligible or compact state
• 12 to 24 months RN experience for Clin II
• 3 years of clinical experience for Clin III
• Diploma or Associates Degree from a professional nursing program recognized and/or approved by the corresponding state's Board of Nursing

Physical Requirements:

• Lifting 20-50 lbs
• Exposure to potentially hazardous and infectious substances
• Prolonged standing, sitting, bending, walking, and climbing

Work Schedule:

This is a Weekend Evening shift position, with individual shift rotation and schedules established with your Nurse Manager/Practice Manager. Schedules may be adjusted as necessary to meet unit requirements and/or as needed.
