Healthcare Career Development Specialist

We are seeking a highly motivated and experienced Case Manager to join our team at Dwyer Workforce Development. As a Case Manager, you will play a critical role in providing comprehensive support to individuals who lack opportunity and aspire to pursue a career in the healthcare industry.
Key Responsibilities- Develop a good working knowledge of our agency's mission, vision, and philosophy.
- Engage with applicants and collect data to determine program eligibility based on our policies, application information provided, and required documentation.
- Maintain follow-up contact with participants through various means, including telephone, mail, email, in-person visits, and text messages.
- Responsible for thoroughly and accurately documenting service encounters with participants, trainers, and provider/employer partners, activities, and interventions, as well as demographic information for improvement and evaluation.
- Maintain current and confidential record-keeping systems, including updating contact notes and tracking the progress of participants in meeting training and employment goals.
- Work with participants to identify barriers to success, use self-sufficiency development tools and resource solutions, and action plans to eliminate barriers.
- Provide direct services to participants in crisis situations, including supportive listening, community and resource referrals, goal-setting, and appropriate follow-up.
- Secure employment opportunities for participants through job development activities and effective communication with provider/employer partners.
- Assist participants in developing and honing job-seeking and workplace readiness skills in preparation for successful employment.
- Provide follow-up services to support job retention, as well as ongoing engagement opportunities and support to facilitate independence, assisting participants in maintaining life balance.
- Knowledge of local community resources and social services in the applicable geographic area.
- Bachelor of Arts or Science in social work or human services or related equivalent experience.
- Previous experience working with human service agencies/programs preferred.
- Possess strong verbal and written communication skills, as well as leadership and motivational skills.
- Proactive qualities with a willingness to contribute and learn.
- Strong time-management skills and an ability to organize and coordinate multiple projects.
- Proficiency with office productivity tools and an aptitude for learning new software and systems.
- Effective organizational development and critical thinking skills.
- Knowledge of Salesforce or other CRM.
- Ability to work flexible hours and evenings, when necessary.
- Ability to establish and maintain effective working relations with a diverse population.
- Bilingual preferred.
Driver's license and access to a vehicle required.
Position LocationRio Grande Valley, Texas.
