As a vital member of our home health team, the Registered Nurse Case Manager (RNCM) plays a crucial role in delivering high-quality patient care. Our RNCMs are dedicated to providing support across a wide range of services, ensuring comfort and dignity for our clients.

Key Responsibilities:

  • Conduct in-person patient assessments and thorough physical evaluations.
  • Oversee the execution and continuous evaluation of individualized care plans, coordinating with home health aides, LPNs, RNs, and other support staff.
  • Communicate effectively with healthcare providers to adapt care as patient needs change.
  • Educate patients and their families on essential home health practices, including wound care, IV administration, and medication management.
  • Work proactively to reduce hospital readmissions by promoting preventive care and ensuring seamless continuity of care.

Qualifications:

  • Current, unrestricted RN license in the relevant state.
  • Valid CPR/AED/BLS/First Aid certification.
  • Reliable transportation for travel to care sites.
  • Experience with trach and/or ventilator care is preferred but not mandatory.
