As a Registered Nurse Case Manager (RNCM), Your Responsibilities Will Include:
- Conducting in-person patient interviews and thorough physical assessments
- Overseeing the execution and continuous evaluation of the patient's care plan by managing home health aides, LPNs, RNs, and other caregivers
- Communicating patient conditions and collaborating with healthcare providers to adapt care as patient needs change
- Educating patients and families on effective home health care practices, including wound care, IV administration, and medication management
- Working to reduce hospital readmissions by promoting preventive care and ensuring seamless continuity of care
Qualifications for the Registered Nurse Case Manager (RNCM) Role:
- Current, unrestricted license to practice as a Registered Nurse (RN) in the relevant state
- Valid CPR/AED/BLS/First Aid certification
- Reliable transportation for travel to care sites
- Experience with trach and/or ventilator care is preferred but not mandatory
