Responsibilities:- Conduct thorough psychiatric evaluations and assessments to identify patient needs
- Develop and implement individualized treatment plans to address patient goals and objectives
- Prescribe and manage medications to ensure optimal patient outcomes
- Provide therapy and counseling to patients to promote mental wellness and resilience
- Collaborate with other healthcare professionals to provide holistic patient care
- Maintain accurate and up-to-date patient records to ensure continuity of care
- Valid Nurse Practitioner license with Psychiatric-Mental Health specialization
- Experience in dementia care, pain management, and outpatient settings
- Proficiency in electronic health record systems, such as Insync
- Ability to provide patient care in a community Mental Health care setting
- Strong communication and interpersonal skills to work effectively with patients and colleagues
- Familiarity with laboratory procedures and diagnostic tests to inform patient care
- Comfort working in a fast-paced clinical setting with a focus on patient-centered care
