Job Summary: We are seeking a highly skilled Licensed Vocational Nurse to join our team at CommUnityCare Health Centers. As a Licensed Vocational Nurse, you will play a vital role in providing routine nursing care and assisting in determining the predictable healthcare needs of patients.

Responsibilities:

* Utilize a systematic approach to provide individualized goal-directed nursing care, collecting and analyzing data to inform patient care decisions.
* Collaborate with patients, families, and healthcare teams to identify health status and needs, developing effective plans of care that prioritize patient outcomes.
* Implement teaching plans for patients with common health problems, utilizing evidence-based practices to promote health education and self-management skills.
* Participate in evaluating patient responses to therapeutic interventions, making adjustments as necessary to optimize patient outcomes.
* Assist in identifying patient needs for referral to resources that facilitate continuity of care, ensuring seamless transitions across healthcare settings.

* High level of skill in building relationships and providing exceptional customer service, fostering positive interactions with patients, families, and colleagues.
* Ability to report and document assessment data accurately, utilizing electronic medical records (EMRs) and other healthcare technologies effectively.
* Strong problem-solving skills, with ability to analyze complex patient situations and develop effective solutions that prioritize patient well-being.
* Excellent verbal and written communication skills, with ability to communicate effectively with patients, families, and healthcare teams.

Requirements:

* Current licensure as a Vocational Nurse in the State of Texas.
* Completion of a healthcare provider Cardiopulmonary Resuscitation (CPR) course, with maintenance of current certification throughout employment.
* High school diploma or equivalent, with experience working in healthcare settings preferred.

Estimated Salary Range: $65,000 - $85,000 per year, depending on experience and qualifications.
