About the Role:

We are seeking a dedicated and compassionate Hospice Registered Nurse to join our team at Gentiva Hospice. As a Hospice RN, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.

Key Responsibilities:

Provide direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load.
Act as primary liaison between physician, patient/family and hospice team.
Empower patient's caregivers by educating them on care for their loved one.
Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity.
Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.

Requirements:

RN license in the state of requested employment (or eligibility to obtain).
1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
Current driver's license and automobile insurance.
Current CPR certification.
