Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team in South Richmond. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify patient needs and develop effective care plans.
  • Communicate with patients, families, and healthcare team members to ensure seamless care coordination.
  • Provide education and support to patients and caregivers on disease management, treatment regimens, and medication administration.
Requirements
  • Current LPN licensure in the state of Virginia.
  • Basic Cardiac Life Support (BCLS) certification.
  • 1 year of experience in home health or a related field.
