Position Overview:
The Care Transition Specialist will play a crucial role in managing a diverse set of responsibilities, demonstrating clinical knowledge, and excelling in intricate case management, utilization assessment, and care coordination for vulnerable members during their hospital stay and the establishment of transition plans following discharge.

Key Responsibilities:
This role requires a person-centered approach, emphasizing support and adaptability to optimize member engagement. The focus will be on guiding members through the healthcare continuum to enhance their quality of life and health outcomes while minimizing unnecessary emergency room visits and preventable hospitalizations.

The Care Transition Specialist will deliver in-person, telehealth, and telephonic complex case management to designated members both in healthcare facilities and within the community, ensuring that members receive care tailored to their individual health and life circumstances.

Essential Qualifications:
- A valid Registered Nurse license with a minimum of 3 years of relevant experience in complex case management.
- At least 3 years of varied clinical experience across more than two healthcare settings (such as hospitals, skilled nursing facilities, home health, or rehabilitation services).
- Willingness to travel frequently to local healthcare facilities or community organizations to engage with members and foster relationships with staff.
- A Bachelor’s degree along with Case Management or Care Coordination certification (CCM or CCTM) is preferred.
