Job Summary

The RN Case Manager Lead will oversee a team of RN Case Managers in the inpatient and emergency departments at NCH Healthcare System. This role requires strong leadership and team-building skills, excellent communication skills, and problem-solving abilities.

Key Responsibilities
  • Support the Director of Case Management and Social Work Manager in implementing and enforcing NCH policies and procedures.
  • Supervise, orient, and mentor RN Case Managers.
  • Ensure comprehensive individualized discharge planning that meets patient needs.
  • Identify gaps in services and develop strategies to improve patient care and resource utilization.
  • Participate in quality of care initiatives, length of stay, readmissions, and avoidable days.
  • Provide patients/families with resources to meet medically related social and emotional needs.
  • Ensure safe transition of patients from one care environment to another.
  • Promote patient/family involvement and ensure choice in discharge planning.
  • Collaborate with multi-disciplinary teams, healthcare providers, and community partners to ensure comprehensive patient care and care transition.
Requirements
  • Minimum of Associate Degree in Nursing required; BSN preferred.
  • A minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management is required.
  • Licensed as a Registered Nurse in the state of Florida.
  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
  • Case Management Certification Preferred.
  • Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
