Crisis Center Case Manager

Job Summary:
We are seeking a highly skilled and compassionate Direct Care Staff to join our team at Indian Rivers Behavioral Health. As a key member of our crisis center, you will play a vital role in providing exceptional care and support to individuals in need.
Key Responsibilities:
- Assess and develop comprehensive discharge plans for clients, ensuring a smooth transition to aftercare services.
- Collaborate with a multidisciplinary treatment team to provide holistic care and support.
- Cultivate relationships with community resources to ensure seamless referrals and coordination.
- Communicate effectively with clients, families, and external agencies to ensure timely and accurate information exchange.
- Provide crisis intervention and support to clients, adhering to established protocols and guidelines.
- Ensure accurate and up-to-date client charts, maintaining confidentiality and adhering to HIPAA regulations.
- Participate in regular team meetings and training sessions to enhance skills and knowledge.
Requirements:
- Bachelor's degree in a mental health or human services related field.
- ADMH case management training/certification.
- Valid Alabama driver's license and good driving record.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
