Job Title: Clinical Manager RN

Ashby Ponds by Erickson Senior Living is seeking a skilled Clinical Manager RN to oversee the care performance of teams within Memory Care and provide clinical care at the highest practicable level using the Erickson Person-Center Approach model of care.

Key Responsibilities:
  • Ensure compliance with state, local, and company policies, procedures, and regulations.
  • Coordinate and provide clinical oversight and care to residents within Assisted Living and Memory Care, including medication management, pain management, wound care, nutrition, and respiratory care.
  • Partner with Assisted Living Manager to complete resident pre-screening/admission, re-admission, and ongoing assessments.
  • Develop and maintain resident individualized service plans in collaboration with other assisted living/memory care team members.
  • Coordinate and monitor outside support services, such as hospice, therapy, and home health, following established plans of treatment.
  • Track and analyze clinical quality areas, including wound care, infection control, falls risk reduction, pain management, weight loss, and dehydration.
  • Participate in continuing care meetings, including Operations Meeting, Service Plan Meetings, Family Meetings, and Performance Improvement/Risk Management/Safety Committee.

Requirements:
  • Current RN license in the state of operation.
  • A minimum of 5 years of experience as a registered nurse.
  • Management experience within a Home Health, Assisted Living, and/or Memory Care licensed setting preferred.
  • Experience and comfort working with an electronic medical record (EMR) preferred.
