Job Title: Clinical Services Specialist

Location: Northridge School District

We are seeking a skilled Clinical Services Specialist to provide high-quality clinical services for referred patients in school and/or clinic settings. As a key member of our team, you will offer consultation to area schools and agencies, providing treatment and/or care coordination of mental health issues.

Clinical Duties:

  • Deliver clinical services for referred patients in school and/or clinic settings.
  • Offer consultation to area schools and agencies.
  • Provide treatment and/or care coordination of mental health issues.
  • Conduct clinical assessments to identify patient needs.
  • Develop and implement integrated wellness plans consistent with principles of client self-management.
  • Facilitate behavioral interventions to support patient well-being.
  • Collaborate with agency staff and community service providers to coordinate referrals and delivery of services.
  • Develop training skills and patient education strategies to promote healthy behaviors.

Medical Administrative Duties:

  • Maintain complete medical records, including history, physical exams, and telephone message summaries.
  • Complete progress notes in each assigned work area and other special patient forms.
  • Participate in health collaborative activities to improve patient outcomes.

Administrative Duties:

  • Carry out medical administrative functions necessary for quality patient care and compliance with accreditation requirements.
  • Participate in committee activities and chart reviews.
  • May participate in Quality Improvement and Risk Management activities and committee work in assigned areas.

Salary Range: $60,000 - $80,000 per year.
