As a Home Health LPN with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to providing comprehensive care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities
• Provide skilled nursing interventions to patients in their homes, working under the direction of a RN and in compliance with state and federal regulations.
• Develop and implement individualized care plans that address the unique needs and goals of each patient.
• Collaborate with the healthcare team to ensure seamless care coordination and communication.
• Maintain accurate and detailed records of patient interactions and care provided.
Requirements
• Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
