Job Summary:

The Diversion Specialist will work closely with our clients, community partners, and internal teams to provide comprehensive support services and referrals to help individuals and families achieve stable housing and self-sufficiency.

Key Responsibilities:
  • Conduct initial intake interviews and assessments to identify client needs and develop personalized service plans.
  • Provide referrals and connect clients with community resources, including Homebase, SSVF, and other partner agencies.
  • Collaborate with internal teams to ensure seamless service delivery and client support.
  • Advocate for clients to access entitlements and negotiate service delivery systems.
  • Engage clients in problem-solving dialogue to identify and access additional resources.
  • Provide leadership and direction in the area of housing for all residents.
Requirements:
  • Bachelor's degree in social work or a related field, or equivalent experience and skills.
  • Prior experience in case management, assessment, counseling, and crisis intervention.
  • Knowledge and understanding of Adult Systems approach to practice and team concepts.
  • Ability to provide evening and weekend coverage as needed.
