Position Overview:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team under the guidance of the Vaccine Administration Manager. The ideal candidate will be responsible for administering vaccines to patients of all ages, with a strong focus on pediatric care. Previous experience in a hospital or medical office setting is preferred.

Key Responsibilities:

  • Review patient medical records to confirm vaccination orders and address any discrepancies with the ordering provider.
  • Retrieve and verify the correct vaccines from storage, ensuring accuracy in dosage and type.
  • Identify patients accurately in the examination room using their name and date of birth.
  • Administer vaccines as per the established protocols.
  • Dispose of medical and hazardous waste in accordance with OSHA regulations and clinic policies.
  • Document vaccine details, including type, manufacturer, lot number, and administration site in the Electronic Health Record (EHR).
  • Provide patients and guardians with the latest Vaccination Information Sheet (VIS).
  • Update VIS as required by the CDC and communicate changes to the Medical Assistants at satellite locations.
  • Review the Citywide Immunization Registry (CIR), generate lists of required vaccines, and coordinate recalls with Front Desk staff.
  • Maintain and update the CIR with patient information and scan vaccination records into the Electronic Medical Record (EMR).
  • Monitor and log refrigerator and freezer temperatures daily, ensuring compliance with safety standards.
  • Assist Medical Assistants at satellite sites with inquiries and conduct compliance visits as needed.
  • Prepare Excursion Incident reports in collaboration with other LPNs and Medical Assistants.
  • Conduct monthly inventory checks of vaccines and report any expired items.
  • Report expired or spoiled vaccines in the CIR and arrange for their proper disposal.
  • Input newly received vaccine information into the EMR, including name, manufacturer, lot number, expiration date, and quantity.
  • Adhere to strict infection control protocols to maintain a safe clinic environment.
  • Follow sharps disposal policies and demonstrate understanding of safety procedures.
  • Participate in training sessions with clinical staff to uphold quality improvement standards.
  • Implement recommendations from the Quality Improvement Team and Infection Control Committee as directed.
Qualifications:

  • Fluency in both English and Spanish is essential.
  • A minimum of two years of clinical experience is required, with pediatric experience being mandatory.
  • An active and unrestricted New York State LPN license is required.
  • Experience with Electronic Medical Record systems is preferred.
