*Full-time | 64-80 hours per pay period (Your Choice)| Monday-Friday | 8 am – 4:30 pm
*Qualifications and Experience Requirements
  • Must possess a vehicle with necessary insurance and a valid state driver's license for client home visits.
*Deliver clinical support and case management for hospice patients and their families.

  • Oversee and guide the Hospice Aide.
  • Maintain regular communication with the patient's physician, the Interdisciplinary Group (IDG), and the patient/family regarding the care plan, pain management, and any emerging concerns.
  • Collaborate in developing and executing the personalized care plan, working closely with the patient/family and IDG, while ensuring safety and quality in compliance with current federal, state, and organizational regulations and policies.
*Key Responsibilities of the Hospice RN: * *Manage Care for Hospice Patients* - the Hospice RN is equipped to handle all types of visits and responsibilities, including but not limited to admissions, routine/PRN visits, re-certifications, discharges, and end-of-life care. Assist patients and families in formulating a tailored care plan that aligns with their individual objectives. Execute all patient care duties while considering the specific needs related to the patient/family's age and cultural/spiritual beliefs. Exhibit proficiency in comfort medications, hospice standing orders, and their administration and indications. Supervise the hospice aide at least bi-weekly. Participate in hospice community initiatives as feasible. The Hospice RN Case Manager is on-call and responsible for providing emergency coverage during weekends, evenings, and holidays.
* *Active Participation in the IDG* - Continuous communication and collaboration with all relevant members of the patient's care team and the interdisciplinary team is a vital role of the Hospice RN. Initiate and engage in care conferences to discuss multidisciplinary team responsibilities, the patient's care plan, pain management, bereavement support, new challenges, goals of care, etc. Ensure that IDG notes and updates are completed promptly, and that meeting summaries are clear and contain pertinent nursing eligibility information.
* *Responsibility and Leadership* - the Hospice Nurse demonstrates accountability for daily tasks, expectations, and team standards. Cultivates and promotes a sense of teamwork, pride, trust, and dedication.
* *Must possess a vehicle with necessary insurance and a valid state driver's license for client home visits.

  • Must have sufficient computer skills.
  • Capable of effective verbal and written communication as required by the role.
  • Required to have the ability to read, write, and speak English proficiently to communicate with staff, clients, and visitors.
