Job Description:
  1. Perform comprehensive health evaluations and develop personalized care plans for patients with complex needs.
  2. Provide education and guidance to patients and their families on safe practices and preventive measures.
  3. Collaborate with the healthcare team to ensure seamless patient care and optimal outcomes.
Requirements:
  • Bilingual proficiency in Armenian is essential for this role.
  • A minimum of 1 year of relevant experience, preferably in home health care, is required.
  • Graduation from an accredited Nurse Practitioner program and current Board Certification are mandatory.
Benefits:

The annual salary range is $135,000 - $140,000, including potential sign-on bonuses and discretionary awards. Comprehensive benefits include medical, pharmacy, dental, and vision coverage, 401(k) retirement savings plan, paid time off, tuition reimbursement, and employee assistance program.
