As a key member of our multidisciplinary team, you will play a crucial role in delivering top-notch behavioral health services to individuals grappling with substance use disorders.
Key Responsibilities- Assist in the development of comprehensive social work and psychosocial assessments of patients and families, with a focus on substance use issues.
- Collaborate with a diverse team of professionals to create and coordinate personalized treatment plans, addressing both substance use and co-occurring mental health diagnoses.
- Provide ongoing substance use services, including individual and group counseling sessions using evidence-based practices.
- Advocate for patients and families, assisting them in obtaining entitlements and community services.
- Document patient interactions and treatment plans accurately and in a timely manner.
- Evaluate patient progress and adjust treatment plans as needed.
- Communicate effectively with patients, families, and other caregivers to ensure understanding of substance use issues, entitlements, and community resources.
- Identify and provide referrals for social determinants of health concerns.
- Maintain productivity levels sufficient to meet department goals.
- Participate in departmental activities, such as utilization review, staff meetings, and education programs.
- Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification required.
- High School Diploma or equivalent required; Bachelor's Degree preferred.
- Minimum of one year of training and/or experience in integrated mental health and substance abuse treatment required.
- Demonstrated skill or knowledge in substance use assessment and diagnosis, principles and practices of harm reduction, and application of stage-wise treatment and motivational interviewing strategies required.
- Computer literacy, including electronic health records, word processing, email, internet research, and data entry required.
- Excellent clinical writing skills required.
- Bilingual in English and Spanish preferred.
