Job Title: Social Worker - Behavioral Health Specialist

About the Role:

This is a unique opportunity to work in a hybrid role, combining on-site, community-based, and remote work. As a Social Worker - Behavioral Health Specialist, you will work with the CVS Accountable Care Mt. Sinai team to provide comprehensive, clinical assessments and interventions to patients with complex chronic illness and/or behavioral health needs.

Responsibilities:
  • Conduct comprehensive, clinical assessments of patients to identify unmet basic needs, social and emotional needs, and connectivity with and experience of care.
  • Develop and implement intervention strategies to address basic needs, connectivity to resources, experience of care, and experience of illness.
  • Collaborate with collateral contacts, including medical and mental health providers, to ensure comprehensive care coordination.
  • Provide appointment accompaniment, community and home visits, and telephonic follow-up to support patients in achieving their health goals.
Requirements:
  • LMSW or LCSW in the state of New York.
  • Exceptional skills in engaging patients/families and utilizing critical thinking skills in assessment and coordination of resources within families and hospital and community networks.
  • Ability to develop, maintain and monitor connectivity to medical/behavioral health care, safety and wellbeing of patients.
Preferred Qualifications:
  • Healthcare and/or community-based experience working with patients with complex chronic illness and/or behavioral health population.
  • Proficiency in MS Office and ability to learn hospital documentation and scheduling systems and care coordination applications.
  • Bilingual (English/Spanish) preferred.
