LPN Medication Care Manager

Sunrise Senior Living is seeking a highly skilled LPN Medication Care Manager to join our team. As a key member of our care team, you will be responsible for providing high-quality care and services to our residents by administering medication and treatments in a safe and organized manner.
Responsibilities:- Administer medications and treatments to residents in a safe and organized manner.
- Document medication administration and resident care in accordance with Sunrise policies and procedures.
- Conduct Service and Health Updates as directed by the Resident Care Director.
- Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
- Collaborate with the care team to ensure seamless care delivery and resident satisfaction.
- LPN license in good standing.
- Previous experience working with seniors and administering medications.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and proficiency in electronic medical records.
