LPN Medication Care Manager

At Sunrise Senior Living, we are committed to providing high-quality care and services to our residents. As an LPN Medication Care Manager, you will play a vital role in ensuring the safe and effective administration of medications to our residents.
Responsibilities:- Administer medications and treatments to residents in a safe and organized manner.
- Review and document medication administration records to ensure accuracy and compliance with regulations.
- Collaborate with the Resident Care Director and Wellness Nurse to ensure medication updates and changes are implemented promptly.
- Conduct regular audits to ensure medication carts are stocked and organized.
- Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs) to ensure resident-centered care.
- Provide education and training to residents and caregivers on medication management and safety.
- Contribute to the overall engagement and satisfaction of residents and families through excellent customer service and communication skills.
- Current LPN licensure in the state of employment.
- Previous experience working with seniors and in a healthcare setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and proficiency in electronic medical records.
