Key Responsibilities:
  • Collaborate with healthcare teams to develop and implement personalized care plans that meet the unique needs of patients.
  • Act as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
  • Monitor patient progress, identify potential issues, and develop strategies to address them.
  • Work closely with healthcare providers to ensure timely and effective delivery of care services.
  • Participate in quality improvement initiatives to enhance the overall patient experience and outcomes.
Requirements:

Education and Experience:
  • Bachelor's degree in nursing or a related field.
  • Minimum two years of experience in a healthcare setting, with a focus on case management or care coordination.
Skills and Qualifications:
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Strong analytical and problem-solving skills.
  • Knowledge of healthcare policies and procedures.
Preferred Qualifications:
  • Certified Case Manager (CCM) certification.
  • Experience with electronic health records and other healthcare software.
