Job Summary:

As a Cardiology Case Manager at Kaiser Permanente, you will play a vital role in ensuring seamless patient care and coordination. This position requires strong communication and problem-solving skills, as you will act as a liaison between patients, community resources, and Cardiology medical staff. Your primary responsibility will be to provide continuity of care to patients by coordinating care and services needed in all settings of the continuum of care.

Key Responsibilities:
  • Identify, coordinate, and evaluate administrative and operational activities of services to effectively manage patients' medical needs.
  • Act as a medical liaison between patients, community resources, and Cardiology medical staff, ensuring effective communication and coordination of care.
  • Monitor and evaluate outcomes of patient treatment, devising individualized plans of care to ensure completion of treatment as recommended by physicians and other healthcare providers.
  • Track and manage patients' care and treatment, including referral tracking, requests for outside services from facilities and/or healthcare providers, utilizing available computer resources.
  • Participate in problem-solving and quality improvement monitoring to help develop, implement, and evaluate department-specific processes and policies.
  • Assist in the development of standards, policies, and procedures, as well as educational tools for various patient treatments.
  • Collaborate with physician chiefs and/or clinic supervisors to follow up on issues requiring resolution, updating them on any issues or concerns as necessary.
  • Assist supervisors in maintaining daily operations and serve as a resource to staff, ensuring training for ongoing formal and informal staff development needs.
  • Develop and implement plans/projects to improve operational efficiency and effectiveness, monitoring progress and reporting to supervisors.
  • Initiate and maintain practices and programs that strengthen the delivery of cardiac services.
  • Perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and direct the provision of care on the unit.
  • Provide direct patient care on an as-needed basis, providing services within the scope of licensure and in compliance with all legal, regulatory, and policy requirements relevant to the clinical role performed.
  • Incorporate the KP Nursing Vision, Model, and Values throughout their nursing practice.
Requirements:
  • Minimum two (2) years of direct patient care experience.
  • Minimum two (2) years of case management or care coordination experience.
  • Bachelor's degree in nursing or four (4) years of experience in a directly related field.
  • Registered Nurse License (Hawaii) required at hire.
  • Basic Life Support from the American Heart Association.
Preferred Qualifications:
  • Certified Case Manager (CCM) by the Commission for Case Manager Certification (CCMC).
  • Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications.
  • Masters degree in public health, clinical nursing, or a related field.
