Cardiology Case Manager

As a Cardiology Case Manager at Kaiser Permanente, you will play a vital role in ensuring seamless patient care. Your primary responsibility will be to coordinate and evaluate administrative and operational activities to effectively manage patients' medical needs. You will serve as a medical liaison between patients, community resources, and Cardiology medical staff, providing continuity of care and monitoring plans of care. Your exceptional communication skills and problem-solving abilities will enable you to navigate complex situations and provide ongoing support to patients and healthcare providers.
Essential Responsibilities:
- Coordinate and evaluate administrative and operational activities to effectively manage patients' medical needs.
- Act as a medical liaison between patients, community resources, and Cardiology medical staff, providing continuity of care and monitoring plans of care.
- Monitor and evaluate outcomes of patient treatment, devising individualized plans of care to ensure completion of treatment as recommended by physicians and other healthcare providers.
- Track and manage patients' care and treatment, including referral tracking and requests for outside services from facilities and healthcare providers.
- Participate in problem-solving and quality improvement monitoring to help develop, implement, and evaluate department-specific processes and policies.
- Assist in the development of standards, policies, and procedures, as well as educational tools for various patient treatments.
- Partner with physician chiefs and/or clinic supervisors to follow up on issues requiring resolution, updating them on any concerns as necessary.
- Assist supervisors in maintaining daily operations and serve as a resource to staff, ensuring training for ongoing formal and informal staff development needs.
- Assist with departmental nursing functions as operational needs require.
- Develop and implement plans/projects to improve operational efficiency and effectiveness, monitoring progress and reporting to supervisors.
- Initiate and maintain practices and programs that strengthen the delivery of cardiac services.
- May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and direct the provision of care on the unit.
- Provide direct patient care on an as-needed basis, providing services within the scope of licensure and in compliance with all legal, regulatory, and policy requirements relevant to the clinical role performed.
- Incorporate the KP Nursing Vision, Model, and Values throughout their nursing practice.
Basic Qualifications:
Experience
- Minimum two (2) years of direct patient care experience.
- Minimum two (2) years of case management or care coordination experience.
- Bachelor's degree in nursing or four (4) years of experience in a directly related field.
- Registered Nurse License (Hawaii) required at hire.
- Basic Life Support from American Heart Association.
Additional Requirements:
- Demonstrated knowledge of and skill in adaptability, oral communication, written communication, customer service, conflict resolution, influence, problem-solving, decision-making, teamwork, quality management, and systems thinking.
Preferred Qualifications:
- Certified Case Manager (CCM) by the Commission for Case Manager Certification (CCMC).
- Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications.
- Masters degree in public health, clinical nursing, or related field.
