About the Job Opportunity
We are currently seeking a skilled Healthcare Coordinator to join our team. As a Healthcare Coordinator, you will be responsible for supporting administrative care management activities, including performing outreach, answering inbound calls, and scheduling services.

The estimated salary for this position is $18.00 per hour, which may vary based on factors such as location, experience, and job responsibilities. The expected duration of this contract is 2+ months with possible extension.

About the Role
In this role, you will serve as a point of contact to members, providers, and staff to resolve issues and document member records in accordance with current state and regulatory guidelines. You will also provide outreach to members via phone to support with care plan next steps, community or health plan resources, questions or concerns related to scheduling and ongoing education for both the member and provider throughout care/service.

Key Responsibilities

- Make calls to our members to verify their home care services and document member records in accordance with current state and regulatory guidelines.
- Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services.

Requirements
- High School diploma or GED required.
- 1-2 years of related experience preferred.
