Doral Health & Wellness is a leading multi-specialty practice in Brooklyn, New York City, seeking a skilled Healthcare Coordinator Specialist to join our team. As a senior member of our healthcare team, you will play a critical role in delivering high-quality, patient-centered care.
About the PositionThe ideal candidate will have a strong background in healthcare coordination and experience working with diverse patient populations. You will be responsible for managing patient cases, advocating for needed services, and providing exceptional support to patients.
Key Responsibilities- Develop and implement personalized care plans for patients.
- Maintain accurate records of patient progress and outcomes.
- Collaborate with healthcare providers to ensure seamless care transitions.
- Provide education and support to patients and their families.
- Bachelor's Degree in a related field with 2-3 years of experience in healthcare coordination required.
- 2-3 years of experience in case management and/or care coordination with expertise in providing health/mental health services to clients.
- Bilingual Spanish language skills are essential.
