Licensed Vocational Nurse

ResCare Community Living is seeking a skilled Licensed Vocational Nurse to join our team. As a Licensed Vocational Nurse, you will work under the supervision of a Registered Nurse to provide routine care and general observation of clients' health status.
Responsibilities:- Communicate health and nursing needs to the RN supervisor
- Coordinate admissions, discharges, and follow-up as needed
- Assist in medical and health appointments as required
- Ensure the provision of nursing and health services under the direction of the RN and as ordered by the physician
- Take and record vital signs, weights, wound measurements, and other necessary data
- Identify trends outside of parameters or in need of attention and communicate to the RN or physician
- Report pertinent information and changes in individual health status to the attending physician or health professionals
- Ensure the provision of needed adaptive equipment
- Administer medications, verifying the correct medications are administered and client response is noted and documented
- Work closely with home, facility, or pharmacy staff in consulting on psychotropic medications, side effects, and medication reviews
- Order, review, monitor, and discontinue medications as ordered by the physician
- Assure accurate transcription of monthly Medication Administration Records (MARs) in each individual's record
- Check the medication cabinet on a weekly basis to ensure medications are administered as ordered, documented, and required follow-up occurs
- Return completed med cards at the end of each month, along with expired and unused medications as applicable
- Check program/service areas on a frequent but not less than a monthly basis to ensure necessary medical, nursing, and medication supplies are available and in ample supply
- Report client illness, potential communicable diseases, or infections and work closely with the RN, direct care staff, and other programmatic and healthcare professionals to maintain a safe environment for clients and staff
- Implement isolation precautions as needed
- Ensure health status, care, and services are documented accurately and timely per company and state requirements
- Provide supervision and education for non-licensed staff in the provision of care and services and report any changes
- Complete a quarterly nursing evaluation and review of body systems on assigned individuals per program requirements
- Refer to the physician any assessment findings needing further review
- Ensure incidents are recorded, the individual is safe, receives required medical care, and communicates to the RN and operations leadership timely
- Participate in clinics completed at the operation with PCP, psych, and other healthcare professionals as assigned
- Participate in on-call duties to ensure weekend and after-hours access to clinicians by staff and individuals served
- Participate in quarterly quality improvement activities, weekly team/staff meetings, and Interdisciplinary Team meetings (IDT/PDP)
- Transcribe physician phone orders and other medically related reports
- Completion of a state-approved training program (e.g., diploma, associate's degree, etc. per state requirement)
- Must be a Licensed Practical/Vocational Nurse (LPN or LVN, per state licensure requirements) in good standing
- CPR certification
- Must have at least 6 months of direct nursing experience
- Experience in providing services and supports to individuals with developmental disabilities or related disorders is preferred
- Valid driver's license with a satisfactory driving record, as defined by ResCare's vehicle policy
- Ability to use a computer utilizing company documentation systems
- Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
