Job Title: Licensed Vocational Nurse (LVN)

At the Union of Pan Asian Communities, we are seeking a highly skilled and compassionate Licensed Vocational Nurse (LVN) to join our team. As an LVN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our organization.

Job Summary

The Licensed Vocational Nurse (LVN) will work under the supervision of a Nurse Practitioner and Program Manager to provide treatment to clients with various psychiatric symptom acuities and substance abuse. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Administer medication (including injections) and monitor results, keeping the Nurse Practitioner and/or Psychiatrist informed of changes in clients' conditions.
  • Consult with the multidisciplinary treatment team, including the Psychiatrist, Nurse Practitioner, and primary Clinician, to develop effective treatment plans.
  • Assist with crisis intervention and psychiatric assessments, collaborating with clients, families, and other healthcare professionals as needed.
  • Collaborate with clients' primary care physicians and other healthcare providers to coordinate care and ensure seamless transitions.
  • Refer clients to the Nurse Practitioner or consulting Psychiatrist regarding changes in medication, ensuring accurate and timely communication.
  • Provide education and support to clients and their families regarding medication management and illness management.
  • Perform face-to-face nursing assessments, screens for communicable diseases, and collects medical and medication history.
  • Conduct quality assurance activities related to medication storage and dispensing, ensuring compliance with local and state regulations.
  • Order refills of medications, obtain medication samples from pharmaceutical companies, and dispose of expired medications as required by law.
  • Order medical supplies as needed, dispose of used supplies, and maintain accurate records.
  • Conduct limited physical screenings and provide consultation to staff on physical complaints of clients.
  • Prepare Treatment Authorization Requests (TARs) for the psychiatrist and make arrangements with the pharmacy.
  • Develop and implement health and wellness groups, including smoking cessation, diabetes management, and exercise and mental health programs.
Requirements
  • Bachelor's degree in nursing and current valid license to practice as a Licensed Vocational Nurse in California.
  • Current CPR certification.
  • Knowledge and experience working with diverse communities, with the ability to communicate mental health concepts effectively.
Preferred Qualifications
  • Bilingual/bicultural.
  • Demonstrated knowledge of County HHSA BHS requirements, including documentation.
  • Knowledge of Cerner electronic health record systems.
Work Environment

The Licensed Vocational Nurse (LVN) will work in a fast-paced, dynamic environment, spending approximately 70% of their time sitting and meeting with others or working at a desk and/or computer. The remaining 30% will be spent standing or walking within the work area or outside the office at meetings and events. The ability to communicate in writing and verbally is essential, as is the ability to lift and/or move up to 25 pounds occasionally.
