About the Role

We are seeking a highly skilled Pediatric Behavioral Health Care Specialist to join our team at AdventHealth Central Florida. As a key member of our care coordination team, you will play a vital role in connecting children and adolescents with the services and resources they need to thrive.

Key Responsibilities
  • Conduct comprehensive psycho-social assessments to identify the needs of patients coping with mental health, developmental/behavioral disorders, and substance use disorders.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Collaborate with community organizations, schools, and healthcare providers to ensure seamless transitions and optimal outcomes.
  • Provide education and support to patients and families to empower them to manage their care and achieve their goals.
  • Advocate for patients to access necessary resources and services, including housing, income, and insurance.
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work or a related field.
  • At least two years of experience in social work or behavioral health.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Knowledge of community resources and services, including those related to mental health, substance use, and housing.
