PRN Speech Language Pathologist

KEY RESPONSIBILITIES
The Speech Language Pathologist (SLP) PRN is responsible for delivering high-quality, comprehensive therapy services in the comfort of the patient's residence, as outlined in the physician's care plan. The SLP will conduct assessments, provide treatment, and develop educational programs aimed at restoring patients' speech, language, and swallowing capabilities.
PRIMARY DUTIES INCLUDE:
- Assessing, treating, and monitoring patients in accordance with the speech therapy treatment plan, while adjusting therapy goals as necessary.
- Completing thorough and timely clinical documentation, ensuring accurate charge entry and understanding of reimbursement processes.
- Effectively communicating with patients, families, and the healthcare team, while providing education to all stakeholders.
- Enhancing clinical skills through continuous education, literature review, and the application of evidence-based practices, while mentoring colleagues.
- Aligning personal and professional goals with organizational objectives to support system-wide success.
- Evaluating patients' comprehensive needs, including medical, psychosocial, and environmental factors.
- Conducting comprehensive OASIS Initial Assessments and managing opening documentation when serving as the primary discipline.
- Collaborating with physicians and patients to create and implement personalized Plans of Care that address the patient's evolving needs.
- Setting realistic, measurable goals for patient progress and regularly assessing achievement of these goals.
- Supervising Home Health Aides when appropriate and providing clear instructions for care tasks.
- Maintaining consistent communication with Home Health Aides and documenting supervisory visits at least bi-weekly.
- Following the established patient care plan to ensure task completion.
- Facilitating open communication with patients and their representatives.
- Exceptional interpersonal skills, including strong verbal and written communication abilities.
- Demonstrated expertise in relevant clinical areas as specified for each practitioner.
- Proven ability to foster collaboration and leadership within the healthcare team, ensuring outstanding service delivery to patients and colleagues.
- Proficient in computer systems and standard rehabilitation equipment.
- Meticulous attention to detail.
- Capacity to effectively manage multiple tasks while maintaining high productivity levels.
- EDUCATION: Master's Degree
- EXPERIENCE: Less than 1 Year of Experience
- CERTIFICATION/LICENSE/REGISTRATION:
Speech Language Pathologist (SLP)
Basic Life Support (BLS): BLS certification or completion within 30 days of hire.
