PRN Speech Language Pathologist

ROLE OVERVIEW
The Speech Language Pathologist (SLP) PRN is responsible for delivering high-quality, comprehensive therapy services in the patient's residence as outlined in the physician's care plan. This role involves assessing, treating, and educating patients to enhance their speech, language, and swallowing capabilities.
KEY RESPONSIBILITIES
- Conduct evaluations, implement treatment plans, and monitor patient progress within the speech therapy framework. Adjust therapy goals as necessary.
- Ensure thorough and timely clinical documentation, accurately recording charges and understanding reimbursement protocols.
- Effectively communicate with patients, families, and the healthcare team, providing education and support as needed.
- Enhance clinical expertise through continuous education, research, and application of evidence-based practices, while mentoring colleagues.
- Align personal and professional objectives with organizational goals to foster a cohesive work environment.
- Assess and address the patient's comprehensive needs, including medical, psychosocial, and environmental factors.
- Complete detailed OASIS Initial Assessments and necessary documentation if the primary discipline.
- Collaborate with the physician and patient to create and modify an individualized Plan of Care based on evolving patient needs.
- Set realistic, measurable goals for patient achievement and evaluate progress regularly.
- Supervise Home Health Aides when applicable, providing clear instructions and oversight.
- Maintain regular communication with Home Health Aides to ensure continuity of care.
- Conduct supervisory visits for Home Health Aides at least every 14 days.
- Follow the established patient care plan to ensure task completion.
- Foster open communication with patients and their representatives.
- Exceptional interpersonal skills, including strong verbal and written communication abilities.
- Demonstrated proficiency in designated clinical areas.
- Proven leadership capabilities and synergy with all levels of the healthcare team, ensuring outstanding service delivery.
- Proficient in computer systems and standard rehabilitation equipment.
- High attention to detail and the ability to manage multiple tasks effectively while maintaining productivity.
- EDUCATION: Master's Degree
- EXPERIENCE: Less than 1 Year of Experience
- CERTIFICATION/LICENSE/REGISTRATION:
Speech Language Pathologist (SLP)
Basic Life Support (BLS): BLS certification or completion within 30 days of hire.
