Job Description

Valor Healthcare, Inc. is seeking a skilled Licensed Master Social Worker LMSW to join our team in providing exceptional care to our patients.

As a Clinical Social Worker, you will play a vital role in assessing patients' psychosocial needs, developing treatment plans, and coordinating care with other healthcare professionals.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify patients' strengths and challenges.
  • Develop and implement individualized treatment plans to address patients' needs.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination.
  • Provide education and support to patients and their families on community resources, advance directives, and VA benefits.
  • Document social work interventions and activities in patients' clinical records.

Requirements:

  • Master's degree in Social Work from a program registered by the Department of Education.
  • Valid, unrestricted MSW license in the state of desired employment or any U.S. state or territory.
  • Minimum of 1-3 years' post-master direct counseling experience with adult patients.
  • Certification in Basic Life Support and ACLS.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.
