Location: Birch Healthcare Center

Birch Healthcare Center is seeking a dedicated and compassionate Social Worker to join our team. We are looking for an empathetic professional committed to enhancing the well-being of our residents and providing exceptional support to their families.

Key Responsibilities:

  • Deliver comprehensive social services to residents, advocating for their needs and connecting them with appropriate resources.

  • Support residents and their families in understanding diagnoses, treatment plans, and navigating care options.

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.

  • Maintain accurate and timely documentation in compliance with state and federal regulations.

  • Serve as a liaison to facilitate clear communication between residents, families, and the healthcare team.

  • Assist with the admissions process, providing guidance and emotional support to new residents and their families.

Qualifications:

  • Bachelor’s degree in Social Work or a related field is required; a Master’s degree is preferred.

  • Prior experience in a healthcare or long-term care setting is highly desirable.

  • Excellent communication and interpersonal skills.

  • Strong organizational skills with the ability to manage multiple tasks.

  • Familiarity with state and federal regulations pertaining to social work in healthcare.
