Job Title: RN Case Manager

Job Summary:

The RN Case Manager is responsible for implementing the plan of care for specific patients, beginning on a pre-admission basis and following through placement and into follow-up/after care. This role requires innovative skills for continuous improvement in patient outcomes and cost effectiveness.

Key Responsibilities:
  • Collaborate with the patient, family, Interdisciplinary Care Team members, and external healthcare providers to ensure safe, timely, and efficient care.
  • Initiate and monitor the plan of care, making mid-course changes as necessary.
  • Communicate with internal and external contacts to persuade or gain cooperation and acceptance of ideas.
  • Function as a team leader in the coordination of care to patients and serve as a problem-solving resource.

Requirements:
  • BSN required
  • Minimum of five (3) years of relevant clinical experience required
  • Prior case management experience highly desirable
Compensation:

Salary Range: Minimum $39.94 per hour - Maximum $61.91 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
