We are seeking a skilled RN Care Coordinator to join our team at LifeBridge Health. As a RN Care Coordinator, you will play a critical role in managing individualized patient-centered care through the use of the Nursing Process and Primary Nursing principles.
Key Responsibilities:
- Manage a panel of patients and work with primary care Interdisciplinary Teams (IDT) to coordinate care and achieve desired health outcomes.
- Utilize evidence-based medicine, data analytics, and innovation to implement care management principles.
- Develop and implement plans of care for assigned patients, including performance goals, monitoring, follow-up, and outreach activities.
- Interface with providers, nurses, and community resources to expedite medically appropriate and cost-effective care.
Requirements:
- Graduate of an Accredited School of Nursing with a Bachelor's Degree required, Master's Degree preferred.
- Current/Valid Registered Nurse License in the State of Maryland (or Compact State as applicable).
- Case/Care management certified or knowledge of national care management standards and community resources a plus.
- 3-5 years demonstrated proficiency in acute care nursing, knowledge, and skills.
