Who We Seek:
We are looking for a dedicated and experienced Physical Therapist who is passionate about delivering functional rehabilitation in various settings, including homes, schools, workplaces, and community environments.
You will have the opportunity to engage clients in real-life scenarios, such as cooking in their kitchens, navigating grocery stores, dining out, participating in outdoor activities, and more.
You will excel in an independent work environment while being part of a collaborative team of skilled therapists.

Responsibilities:
As a Physical Therapist, your duties will include but are not limited to:
- Conducting evaluations and creating personalized treatment plans
- Regularly reassessing the effectiveness of treatment plans and participating in staffing meetings and family conferences
- Performing discharge evaluations and making recommendations for home and community safety
- Providing referral and educational resources
- Communicating patient needs and progress to the treatment team, physicians, clients, and family members
- Collaborating with other disciplines to ensure coordinated care and enhance patient outcomes.

Qualifications:
To be considered for this role, you will need:
- A minimum of a bachelor's degree in Physical Therapy from an accredited program
- A current, unrestricted state license as a Physical Therapist
- Current Basic Life Support or Cardiopulmonary Resuscitation certification as per state regulations
- At least one year of experience as a Physical Therapist is preferred
- Knowledge of rehabilitation techniques related to complex neurological injuries
- Effective communication skills in both verbal and written formats
- The ability to lift up to 50 pounds and assist with the moving, lifting, or transferring of patients, which may involve lifting up to 100 pounds while adhering to safety protocols
- Proficiency in fine motor skills, visual acuity, and the ability to stand or walk for extended periods.

Additional Physical Requirements:
This role may require frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
A health screening or examination may be necessary prior to assignment and periodically thereafter, based on specific location policies or local/state regulations to ensure physical capability for assigned duties with or without reasonable accommodations.
