Healthcare Case Manager Position in Home Setting

We are actively seeking a skilled Registered Nurse to fill the position of Healthcare Case Manager in our home health department. This is an exciting opportunity for a dedicated professional to join our team and make a meaningful difference in patients' lives.
In this role, you will be responsible for developing and implementing personalized care plans, collaborating with healthcare teams, and providing exceptional patient-centered care. About the RoleThis full-time position is available to work various shifts, including day, night, and mid-shifts. You will have the opportunity to work with a dynamic team, cross-train in different areas, and contribute to the development of our patients' care plans. Responsibilities- Develop and implement individualized care plans that address patients' unique needs and goals
- Collaborate with multidisciplinary teams to ensure seamless care coordination and efficient resource allocation
- Conduct regular patient assessments, identify potential risks, and develop strategies to mitigate them
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal outcomes
- Bachelor's degree in Nursing or a related field from an accredited institution
- Active and unencumbered Registered Nurse license in the state of Massachusetts
- Minimum 1 year of experience in case management, nursing leadership, or a related field
The estimated annual salary for this position ranges from $75,000 to $150,000, depending on experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package, including:
