Wellness Case Manager

InGenesis is seeking a skilled LVN Wellness Case Manager to join our team. As a key member of our healthcare staff, you will play a vital role in evaluating, planning, and providing case management services for patients with complex chronic conditions.
Key Responsibilities:
- Perform complex case management services, including health education and counseling, to patients with chronic conditions.
- Make appointments and send reminders for regular and ancillary visits, as well as preventive care.
- Utilize a registry to target patients for chronic disease care management activities, such as ensuring patients receive regular visits and preventive care.
- Assist with eligibility determination to ensure access to services.
- Maintain regular communication with other clinical staff involved in panel management.
- May be required to work at other BACH locations within a 50-mile radius to support patient and client objectives.
Requirements:
- 1–2 years of work-related experience in a medical environment.
- Active phlebotomy certificate/blood draw certification.
- Proficiency in Microsoft Office and the internet is a MUST.
- Preferred education, experience, training:
- Associate or bachelor's degree in a related field.
- Experience in a community health center, long-term care facility, or public health setting.
- Diabetes and Hypertension experience.
- Outreach and presentation experience.
Physical Demands:
Sitting, walking, and standing associated with a normal medical/clinical office environment; manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
Traveling to work sites includes some exposure to outside elements. Reading and writing in order to review records for accuracy. May work with blood or blood-borne pathogens and will require OSHA training.
