Nurse Case Manager

Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse - Case Manager to join our team at St Croix Hospice. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and coordinating services to meet the unique needs of our patients and their families.
Responsibilities:
- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Manage patient caseloads, including scheduling appointments, coordinating services, and documenting patient information.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Graduate of an accredited school of nursing.
- Current licensure in the state.
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care preferred.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
