Job Title: Physical Therapy Assistant - Home Care

Providence Health & Services is seeking a skilled Physical Therapy Assistant to join our Home Care team. As a Physical Therapy Assistant, you will work under the guidance of a Physical Therapist to provide therapeutic services and activities to patients in their homes.

Responsibilities:
  • Assist Physical Therapists in providing therapeutic services and activities to patients in their homes.
  • Help patients maintain or improve their level of independence.
  • Work collaboratively with the healthcare team to develop and implement patient care plans.
  • Provide education and support to patients and their families on therapeutic exercises and activities.
  • Document patient progress and communicate with the healthcare team as needed.
Requirements:
  • California Physical Therapy Assistant License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 1 year of experience in Home Health as a Physical Therapy Assistant (preferred).
