Job Overview

This role is an exciting opportunity to join our team as a Medical Care Coordinator. As a key member of our staff, you will assist physicians and other healthcare professionals with daily patient care coordination and access.

You will serve as the liaison between patients, staff, and physicians, maintaining efficient patient flow and delivering direct patient care within the scope of your licensure and in accordance with SMG policy. Additionally, you will collect lab specimens, implement physician orders and nursing procedures, and ensure proper preparation for patient visits.

Key Responsibilities
  • Assist physicians and staff with daily patient care coordination and access
  • Serve as the liaison between patients, staff, and physicians
  • Maintain efficient patient flow
  • Deliver direct patient care within the scope of licensure and in accordance with SMG policy
  • Collect lab specimens and accurately label them
  • Implement physician orders and nursing procedures
  • Ensure proper preparation for patient visits
  • Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager
  • Assure all patients are correctly identified prior to providing and/or documenting care
  • Employ appropriate and timely use of Tasking in EHR
  • Obtain vital signs and document them appropriately in EHR
  • Administer injections according to protocol and document them accurately in EHR
  • Demonstrate telephone triage protocols and document in EHR
  • Schedule appointments accurately and according to department guidelines
  • Adhere to universal safety precautions when administering medications and disposing of medical waste
  • Review and update medication lists in EHR
  • Enter orders and schedule appointments in the Practice Management System (PMS) and EHR
  • Provide referrals and consult orders appropriately and in a timely manner in the PMS and EHR
  • Check and maintain logs for emergency equipment according to policy
  • Facilitate transition of care to UCC/Hospital
  • Maintain proficiency with department-specific equipment and troubleshoot malfunctions promptly
Requirements
  • Associates degree required; Bachelor's degree preferred
  • New Jersey State Nursing License and BLS certification required
  • 0-1 year experience; 2-4 years experience preferred
  • Ability to communicate in English, both orally and in writing
  • Strong interpersonal and organizational skills
  • Ability to perform diverse work assignments with time limitations with a high degree of accuracy
  • Ability to use problem-solving, critical thinking, and priority-setting skills
  • Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred
  • Experience with Standard Office Technology in a Window-based environment preferred
  • Experience with patient care equipment preferred
  • Experience with EHR, Practice Management System, and Departmental/Ancillary Systems preferred

$65,000 - $85,000 per year, depending on experience
