Role Summary

We are seeking Social Workers with a Bachelor’s or Master’s degree (or a related field such as Gerontology) to join our Care Management Team (CMT). This team comprises both Master’s and Bachelor’s prepared Social Workers and Registered Nurses, working collaboratively to deliver thorough case management services to our target demographic.

Ideal Candidate:

The ideal candidate is a passionate Social Worker (BSW) with a commitment to positively impacting individuals' lives. You should be detail-oriented, proficient in computer usage, and eager to collaborate within a management team overseeing the care of clients receiving home and community-based services. You possess strong problem-solving skills, are knowledgeable about community resources, and enjoy assisting clients in addressing social determinants of health care.

You appreciate a workplace that recognizes the vital contributions Social Workers make to client health and welfare, and you prefer a schedule of 8 am to 5 pm on weekdays, with no weekend obligations.

Key Responsibilities:

  • Assist the team in identifying community resources (e.g., housing, medical supplies, primary care providers, essential items like food and clothing, medical transportation, and caregivers) to meet participant needs.
  • Engage in advocacy to ensure participants receive timely medical services for community safety.
  • Respond promptly to inquiries from CMT, participants, and other stakeholders to facilitate seamless patient care coordination.
  • Educate participants about the benefits of available services and community support.
  • Assist participants with applications (e.g., Medi-Cal, IHSS, Calfresh, Regional Center) and explain eligibility criteria; coordinate transportation and other essential support needs.
  • Document all interactions with consumers and maintain timely reporting and documentation.
  • Observe participant behavior, report significant changes to supervisors or the team, and document appropriately.
  • Participate in daily case conferences with CMT regarding shared participants.
  • Perform additional related duties as assigned.

Essential Qualifications:

  • Master's or Bachelor's degree in Social Work or a related field.
  • Proficient in using a personal computer with various standard software applications.
  • Valid California Driver's License and eligibility for insurance under our liability policy with a clean DMV record.
  • Possession of a personal vehicle and proof of current auto insurance.
  • CPR certification required within 30 days of hire.

Preferred Skills and Knowledge:

  • Understanding of the challenges faced by individuals with physical disabilities or emotional disturbances.
  • Familiarity with community and supportive services.
  • Ability to assess and prioritize multiple tasks and projects effectively.
  • Experience in meeting deadlines for project completion.
  • Strong interpersonal skills for maintaining effective relationships with colleagues and community providers.
  • Experience with electronic medical records.

Work Environment:

This position is not remote; work will be conducted from the office. Travel to clients' homes within the service region is required. Personal protective equipment (PPE) will be provided to all employees. Transporting clients in personal vehicles is not permitted.

Schedule:

This is a full-time position, requiring 40 hours per week, Monday through Friday, from 8 am to 5 pm, with a one-hour lunch break. No weekend work is required.

Physical Requirements:

Successful candidates must pass a medical examination that assesses mobility, visual acuity, and hearing acuity. Specific physical requirements include:

  • Ability to sit, stand, bend, lift, walk, and move intermittently during working hours.
  • Capability to lift up to 35 pounds without assistance.
  • Able to drive intermittently.

Compensation and Benefits:

Pay ranges from $22 to $30 per hour, with exact compensation based on skills and experience. Mileage reimbursement is provided for travel from the office to clients' homes. A monthly phone stipend is also included.

Health, dental, vision, and life insurance, along with a Flexible Spending Account (FSA), become available on the first of the month following 60 days of employment. Leave accrues from the first day of employment, allowing for paid vacation, sick, and holiday leave after 90 days. A 401(k) matching program is available after one year of employment and 1,000 hours worked.
