Job Summary

We are seeking a skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a critical role in ensuring the efficient scheduling and delivery of patient care services. This is a full-time position working days, with a competitive hourly rate of $18.00 to $ hourly.

Responsibilities
  • Coordinate patient assessment visits, including Oasis visits, to ensure timely scheduling and delivery of care services.
  • Work collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered and within the care center.
  • Manage calls from patients and field staff related to scheduling issues, referring clinical and performance-related issues to the clinical manager as needed.
  • Monitor hospitalized patients, communicating and ensuring the team follows up as necessary.
  • Support the clinical manager and work collaboratively to ensure timely communication with patients, clinicians, referral sources, business development team, and other office personnel.
  • Run applicable reports and respond to workflow, taking appropriate actions.
  • Assist with internal or external transfer of patients between care centers and/or hospice services.
  • May be responsible for maintaining supply closets with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serving as liaison with the field for patient supply needs.
Requirements
  • High School diploma or GED equivalent.
  • One year of scheduling experience in a healthcare environment, or one year of administrative experience in a healthcare environment.
