ZOLL LifeVest is seeking a skilled Patient Service Representative to train patients on the use of the LifeVest wearable defibrillator.
Job Summary:The ideal candidate will have 1 year of patient care experience and be willing to work as an independent contractor. The Patient Service Representative will be responsible for setting up equipment, training patients and caregivers, and following up with patients as needed.
Responsibilities:- Manage equipment and garment inventory
- Contact caregivers and family to schedule fittings
- Available to conduct evening and weekend activities
- Willing to travel to patient's homes for fittings or follow-up visits
- Disclose family relationship with any potential referral source
- Program equipment according to prescribing physician's orders
- Measure patient and determine correct garment size
- Train patient and other caregivers in the use of the LifeVest
- Have patient sign Patient Agreement and WEAR Checklist
- Fax signed copy of Patient Agreement and WEAR Checklist to ZOLL within 24 hours of assignment
- 1 year of patient care experience
- Patient experience must be professional (not family caregiver)
- Patient experience must be documented on resume
- Willing to have background check completed
- Florida and Ohio candidates must complete level 2 screening, fee not paid by ZOLL
- Disclose personal NPI number (if applicable)
- Have valid driver's license and car insurance
- Willing to pay for additional training and vendor credentials (i.e. DME/RepTrax), fees not paid by ZOLL
