Position Overview

The SOAR Specialist plays a crucial role in providing essential support services to individuals facing mental health challenges. This position focuses on delivering Community Support Specialist and Targeted Case Management services, particularly for those in crisis or at significant risk of involuntary commitment.

Key Responsibilities:
  1. Deliver SOAR, Community Support Specialist, and Targeted Case Management services to assigned individuals, prioritizing those in crisis.
  2. Complete all required training and orientation as mandated by relevant oversight entities.
  3. Collaborate effectively as a member of the Crisis Services Team and other designated treatment teams.
  4. Successfully undergo SOAR training and assist individuals in applying for SSI/SSDI benefits.
  5. Support clients in accessing additional benefits such as housing, healthcare, and medication.
  6. Establish and nurture relationships with external community resources, hospitals, and referral agencies.
  7. Utilize electronic record systems to document client interactions as instructed, including entries into the Homeless Management Information System (HMIS).
  8. Manage a designated caseload and ensure proper documentation of services in compliance with regulatory standards.
  9. Prepare and submit reports or documentation to supervisors as requested.
  10. Meet productivity expectations as outlined by management.
  11. Perform additional duties as assigned.
Qualifications:
  1. A bachelor’s degree in social work with LSW licensure or eligibility for LSW licensure.
  2. Demonstrated ability to engage effectively with individuals experiencing severe mental health issues.
  3. Capacity to work collaboratively within a treatment team to achieve individual and organizational goals.
  4. Possession of a valid driver’s license, automobile insurance, and an insurable driving record throughout employment.
  5. Successful completion of a background check and drug screening, adhering to a drug-free workplace policy.
