Position Summary:
The role involves delivering a range of recovery-focused services to individuals engaged in a crisis stabilization program, aimed at enhancing their independence, community involvement, and recovery from mental health or substance use challenges.
Key Responsibilities:
- Participate in all required training sessions and meetings organized by Communicare and the Department of Mental Health.
- Maintain certification as a Peer Support Specialist.
- Collaborate effectively with other mental health professionals.
- Provide tailored peer support services based on individual needs.
- Assist in the onboarding process for new clients.
- Facilitate transitions for clients to enhance their access to resources and support systems.
- Enhance communication between clients and service providers.
- Foster a deeper understanding of lived experiences, recovery, and resilience by exemplifying personal accountability, self-advocacy, and optimism.
- Keep accurate records of services provided in client files.
- Offer crisis support by actively listening and working with the treatment team.
- Encourage clients to take an active and self-directed approach to their recovery journey.
- Conduct daily group peer support sessions across all units.
- Coordinate discharge planning for clients preparing to exit the program.
- Perform additional duties as assigned by the immediate supervisor or Program Coordinator/Director.
Qualifications:
- High School Diploma or GED; individuals aged 16-20 must be enrolled in school or a GED program.
- Possess reliable transportation, a valid driver's license, automobile insurance, and a safe driving record.
- Identify as a current or former mental health consumer with a sustained period of self-directed recovery, or be a first-degree family member of a current or former consumer.
- Ability to successfully complete training provided by DMH and the Center.
